Verify Insurance Coverage

Below are some of the common billing codes (also known as CPT codes) that are used by our office for different types of visits.

We encourage you to contact your insurance provider to get a better understanding of what your personal out-of-pocket costs could be, based on the health coverage you have. Your medical provider will advise which services you need during your appointment, as you may not need all of the items under the visit category below or you may need additional procedures based on your complaints and your exam.

It is important to understand whether you have a copay, coinsurance, or deductible to be met, and the date that your deductible resets. Sometimes your office visit may be covered by a copay, but your procedure may be applied to your deductible.

We do NOT set the prices for services if you want to use your insurance coverage – this is done by your insurance company. We do offer self pay pricing – please let us know if you would like to be self pay and we can offer you discounted pricing for paying in full at the time of service. In this case we would not submit any claims to insurance on your behalf.

As every insurance plan is different, is the responsibility of the patient to verify insurance coverage and any out-of-pocket costs prior to each visit. We do our best to provide an estimate, but sometimes we are given inaccurate information by your insurance company. In addition, insurance companies have a disclaimer that “Benefits are NOT guaranteed until the claim is submitted”. Should our estimate differ from the actual costs, you are still responsible for payment of all services rendered.
